Above all, our aim is to provide each of our patients with an optimal healthcare experience. We are a recognized patient-centered medical home (PCMH) provider. The PCMH is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient's family. Care is facilitated by Health IT, registries, and health information exchange to assure patients get the best care possible.
